What I heard on the news at the time of this incident was that a truck had driven over this manhole cover and flipped it into the street.
There must have been an edge out already or a rock underneath one corner or something for this manhole cover in the street to fly out of there this way when a truck hit it. It also could have been bent (the lid) from previous heavy load vehicles like trucks and buses too. There is likely no sure way to stop something like this from happening periodically but it is pretty sad the lady didn't see it and died falling about 10 feet through this empty hole under the street.
