Because regarding the public Health the Constitution gives this power to Governors and local authorities. Why?
Because when the government was founded unless you had public health run by the states there weren't even telegraph poles yet. So, it was never practical for the Federal Government to be directly involved with the public health. So, it isn't the president's responsibility or authority in regard to anything regarding the public health because of this.
So, out of practicality this was always left to states and localities to cover problems associated with public health concerns regarding the Constitution. So, this was just set up this way because by the time the Federal government got involved everyone would be dead already in the 1770s and after until the telegraph was invented likely in the mid 1800s.
