Pandemics have historically always emptied the world's cities

 It's always been this way. Why? Because before vaccinations if you didn't leave cities with pandemics you died. Very simple.

So, why wouldn't people leave cities when pandemics occur?

To not leave (in the past before vaccinations) was to die. Period.
